NAMES, SIGNATURES, ETC...
This is a small page. You just have to put names and signatures in.
It says: I ......... on behalf of ....... am applying for eligibility determination for admission.....
You sign it, date it. Oh.. beware here as well. All signatures must be within 1 year of the application being received or it's null and void! Personally, I wouldn't push the one year limit and have the dates as close to the time the application is being submitted as possible. Say within a couple of months if possible.
If it's the applicant themself that is filling in this form they just write... I...... so and so.. on behalf of 'myself'. etc.
